These terms of service outline the agreement between Jennifer Clarke, MSc, RP, and her clients and website users. By accessing these services, you agree to these terms.
Services
Consultations: A free 15-minute consultation is offered for prospective clients on topics related to mental health and emotional well-being.
Treatment and Recommendations: Personalized treatment plans and recommendations are developed based on the results of evaluations and ongoing work with clients.
Payment and Billing
Fees for services will be disclosed and agreed upon prior to the provision of services. Clients will be notified of fee changes.
Cancellation and Rescheduling
• Cancellation Policy: Clients must provide at least 24 hours' notice for cancellations or rescheduling of appointments.
Late fees may apply for cancellations or rescheduling with less than 24 hours' notice.
Confidentiality and Privacy
Confidentiality and privacy of client information is maintained in accordance with the privacy policy.
Limitation of Liability
Jennifer is not liable for any damages or losses resulting from the use of these services or website.
Governing Law
These terms of service are governed by and construed in accordance with the laws of the province of Ontario.
Changes to These Terms
Jennifer reserves the right to modify these terms of service at any time. Changes will be posted on this website and effective upon posting.
